29 CFR 1904.33(a): The employer did not maintain the OSHA 300 log, the privacy case list, the annual summary and the OSHA 301 Incident Report forms for five (5) years following the end of the calendar year that those records covered: (a) The Bridge at Colorado Springs at 2494 International Circle Colorado Springs, CO 80910: On or before December 9, 2021 the employer did not keep or maintain OSHA logs as required by this standard. Specifically, the employer did not have OSHA logs for 2016 or OSHA 301 logs for the years 2019 or 2020. Abatement Note: Abatement certification and documentation are required for this item (see enclosed Certificate of Creative Action Worksheet).
