Kytle Street in the city of Cleveland, Georgia: On or about December 7, 2021, an employee was performing DensGlass installation on a store front awing was exposed to fall hazards of approximately 17 feet to lower levels while standing on the guardrails of a scissor lift used as a work platform that was not at least 18 inches wide.

29 CFR 1926.454(a):The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. (a) 271 E. Kytle Street in the city of Cleveland, Georgia: On or about December 7, 2021, an employee utilizing a scissor lift to reach an elevated working area was exposed to fall hazards when the employer did not ensure that the employee was properly trained in the usage of the scissor lift.
