29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: On or about December 07, 2021, at the Akita Drilling Rig #525, the employer did not provide a functioning eyewash station in the mud tank/pit area where employees were mixing corrosive materials such as, but not limited to, Caustic Soda Anhydrous. This condition exposed employees to chemical burn hazards.

29 CFR 1910.28(b)(11)(i): The employer did not ensure that each employee exposed to an unprotected side or edge of a stairway landing that is 4 feet (1.2 m) or more above a lower level is protected by a guardrail or stair rail system. Akita Drilling Rig #525: On or about December 07, 2022, the flow line stairway (catwalk) did not have railings to the last five steps on either side. This exposed employees to fall hazards.
