29 CFR 1910.305(b)(2)(i):Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy: On or about December 6, 2021, employees engaged in drilling operations were exposed to electrical shock hazards due to two outside outlets on trailer 826 were compromised in following instances: a) Right side outlet missing complete cover (weatherproof cover). b) Left side outlet has a broken cover (weatherproof cover).
