29 CFR 1910.147(c)(4)(ii)(A): The Energy Control Procedures did not include a specific statement of the intended use of the procedure: Air Handling Units (AHU) 1-5: The AHU lock out instructions did not specify when they were to be used, such as during scheduled monthly maintenance to ensure that the AHU unit was stopped, isolated from all potentially hazardous energy sources and locked out before employees performed any servicing or maintenance. Employees were exposed to electrical and caught in hazards.

29 CFR 1910.147(e)(3): Specific procedures and training were not developed, documented and incorporated into the employer's energy control program for removal of a lockout or tagout device under the direction of the employer when the authorized employee who applied the lockout or tagout device was not available to remove it: Air Handling Units (AHU) 1-5: The maintenance supervisor was given the extra key during lockout operations and the program did not include specific procedures for removal of locks by the employer. Employees were exposed to electrical and caught in hazards.
