The employer did not establish and implement an accident prevention program where employees were exposed to fall hazards.

Employee was observed installing wood materials using a nail gun without eye/face protection.

Employees were observed accessing and egressing the roof by way of portable ladders that did not extend 3 feet past landing.  Employees were exposed to a fall distance of greater than 14 feet eave to ground.
