Delphos OH 45833, were exposed to injury from pinch points created by moving components from Caterpillar unit 29 forklift (Model: CAT 2P3500 - Serial No. AT3440252). The employer did not ensure that the forklift was locked out prior to employees preforming maintenance and repair on the forklifts mast pulley chain.  The employer did not ensure energy control application steps were implemented to control the machine's hydraulic energy sources prior to the maintenance and repair tasks - the employer did not: 	Turn off or shut down the equipment. 	Physically operate energy isolating devices to control and release energy sources and affix lockout device(s). 	Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe. 	Verify that isolation and deenergization of the machine or equipment had been accomplished
