On or about December 13, 2021 at 332 Summit Street, Manchester, CT job site: Employees using a Bostitch nail gun to construct a new roof were not provided with or required to wear eye protection.

29 CFR  1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about December 13, 2021at 332 Summit Street, Manchester, CT job site:  A 24 foot long aluminum extension ladder was used on the south east corner of the house.  The extension ladder did not extend three feet past the roof eave.
