29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energization, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    Production Area, Castaly Double-Sided Planer (Model No. PL 18S, Serial No. YN201005) and Morgan Band Re-Saw (Serial No. 0607021605).  On or about November 19 through December 14, 2021, employees were exposed to amputation hazards when performing servicing and maintenance activities such as cleaning of wood dust and changing of saw blades.  Employees did not de-energize and lockout the machines prior to performing servicing and maintenance. The employer did not implement and maintain a lockout/tagout program consisting of specific energy control procedures, employee training and periodic inspections as follows:     a) Procedures for the control of hazardous energy were not developed, documented and utilized by the employer.     b) Training of "Authorized", "Affected" and "Other" employees was not conducted and certified by the employer.   Employees authorized to perform lockout/tagout were not informed of and trained to control hazardous energy sources according to specific procedures.  Lockout devices were not provided.  c) Periodic inspections of the energy control procedures were not conducted and certified by the employer at least annually to ensure that the procedures and requirements of the standard were being followed.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment.  Production Area, Near East Exit Door.  On or about December 14, 2021, the employer did not maintain at least 36 inches of clear space in front of electrical disconnect boxes up to 240V.  Access to working space and shutoff in the event of an emergency was partially blocked by storage of a machine cabinet.    There is no abatement documentation or certification required for this item.
