On a third level balcony, located 70 Ascend Circle, Building 12, Saint John's, Florida 32259: On or about December 14, 2021, the employer exposed employees to fall hazards, in that fall protection was not utilized by employees while unloading plywood that was hoisted by a forklift truck to the unguarded balcony.
