On or about December 15, 2021, on the east side of the site, employee(s) engaged in cinder block/mason cutting were not wearing eye protection while using a Stihl 14in. Cutoff Tool Model# TS 420w.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffolds, structure, personnel hoist, or similar surface was not used. On or about December 15, 2021, on the east side of the site, employee(s) were cutting a cinder block using a Stihl 14in. Cutoff Tool Model# TS 420w on a scaffold platform at a height greater than two (2) feet that did not have a means of access.
