An employee was engaged in residential siding activities using a pneumatic nail gun. This employee was exposed to struck-by hazards because he was not utilizing eye protection.

In this particular scenario, the "A: frame ladder was not opened/locked according to the manufactures requirement - The ladder was stowed in the close position and leaning up against the side of the home.
