29 CFR 1926.403(b)(2):Installation and use. Listed, labeled, or certified equipment shall be installed and used in accordance with instructions included in the listing, labeling, or certification. On or about December 14, 2021, the employer did not ensure that flexible cords used by employees were used in accordance with instructions included in the listing, labeling, or certification, exposing employees to fire and shock hazards in the following instances: a) In the basement, multiple flexible cords were plugged together in parallel to power portable temporary lighting, DEWALT 4-port battery charger, and Phoenix Guardian HEPA Air Filtering System (SN E1767441). b) Two flexible cords were plugged together in parallel to power the construction trailer.
