29 CFR  1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:   a) Gill House Inn, 13565 Harbor Road, Henderson Harbor, NY  13651, on the first floor, on or about 12/10/21:  Workers were exposed to electrical burns where a quad outlet was being used and was not mounted to the wall and did not have a cover.  b) Gill House Inn, 13565 Harbor Road, Henderson Harbor, NY  13651, on the first floor, on or about 12/10/21:  Workers were exposed to electrical burns where a light switch (dimmer) being used and was not mounted to the wall and did not have a cover.  c) Gill House Inn, 13565 Harbor Road, Henderson Harbor, NY  13651, in the basement, on or about 12/10/21:  Workers were exposed to electrical burns where a office style power strip was used outside its design and in a permanent application.

29 CFR  1926.602(c)(1)(ii): Modifications or additions affecting the safe operation of the equipment were  made without the manufacturer's written approval:   a) Gill House Inn, 13565 Harbor Road, Henderson Harbor, NY  13651, on or about 12/10/21:  A SkyTrak 42 telehandler was used to lift the steel railing into position.  An aftermarket jib attachment was installed onto the forks that was not approved by the manufacturer.
