29 CFR 1910.22(a)(2): The floor of each workroom was not maintained in a clean and, to the extent feasible, in a dry condition: a.) On or about 12/15/2021, at 330 N. Ingraham Ave, Lakeland, FL, 33801, in Freezer C, ice accumulated near the one way exit curtain strip door, where employees operate power industrial trucks and are exposed to slip, trip/fall and overturn hazards. b.) On or about 12/15/2021, at 330 N. Ingraham Ave, Lakeland, FL, 33801, in the west dock and near the doc plates, the floor was not dry, it had water condensation, which exposes employees working in this area to slip/trip and fall hazards. c.) On or about 12/15/2021, at 330 N. Ingraham Ave, Lakeland, FL, 33801, in the east dock near to Freezer C, the floor was not dry, it had water condensation, which exposes employees working in this area to slip/trip and fall hazards.

On or abut 12/15/2021, at 330 N. Ingraham Ave, Lakeland, FL, 33801, employees were exposed to forklift related hazards, in that, forklift operators did not receive formal training for power industrial trucks but were allowed to operate forklifts, exposing them to crush-by and struck-by hazards.
