29 CFR  1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to an outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket).  On or about December 15, 2021, employees engaged in machining operations were exposed to electrical shock hazards in various locations,  due to the employer failing to visually inspect the flexible cord used to power equipment.  1. Lift Magnet plugged into a frayed extension cord on the south side of the bay (Permadur Model TP-100). 2. Thrity -two-inch Pedastal Shop fan on the northeast side of the shop.
