29 CFR  1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  Lavender Ridge Inc., 1400 S. 34th Street, Mount Vernon, IL 62864  On or about 2/01/2022, the employer had not established a written respiratory protection program that included the provisions of 29 CFR 1910.134(c)(1)(I) - (ix). The employer required employees in the facility to wear an N95 filtering facepiece respirator when entering the rooms of known COVID-19 positive residents to administer care. Employees would enter the room at least 3 times per day for meal delivery, changing of clothes and/or vital sign checks.

The employer required employees in the facility to wear an N95 filtering facepiece respirator when entering the rooms of known COVID-19 positive residents to administer care.
