29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   On or about 12/14/2021, in Lowell, Arkansas in the Wood Creek Loop subdivision in the following instances:  a) 603 Wood Creek where the framer's ladder extended approximately 6 inches above the deck of the second level; b) 607 Wood Creek where the roofer's ladder extended approximately 6 inches above the roof's edge. This condition exposed the employees to the hazard of falling from the structures or ladders as they dismounted or mounted the ladders.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about 12/14/2021, in Lowell, Arkansas in the Wood Creek Loop subdivision, in the following instances:  a) 603 Wood Creek where a carpenter was exposed to the hazard of falling from the second floor; b) 607 Wood Creek where two roofers were exposed to the hazard of falling from the roof; c) 615 Wood Creek  where a roofer was exposed to the hazard of falling from the roof.
