29 CFR 1926.451(e)(1): Cross braces were used as a means of access and egress: On or about 12/15/2021 at the jobsite where an employee was observed climbing onto and working from the inside cross braces of the second platform level of the scaffolds erected on the west end of the building, a practice that exposed him to the hazard of falling from the scaffold.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards, in the following instances: On or about 12/15/2021 at the jobsite where employees working on the scaffold system that had been erected on the northwest corner of the building were exposed to the hazard of falling from the scaffold that did not have fully guarded working platforms in the following instances: a) the midrail was missing from the 3rd platform at the west end of the north side; b) the midrail was missing from the 4th platform at the west end of the north side; c) the midrail was missing from the 5th platform at the west end of the north side; d) the midrail was missing from the 8th platform at the west end of the north side; e-m) one guardrail was placed across the south end of the west platforms 2 through 10 at a 43.5 inch height, instead of two guardrails at 21 and 42-inches respectfully; n-s) one guard rail was placed at a 54.5-inch height on the second through seventh platforms of the east end of the north side scaffolds, instead of two guardrails at 21 and 42 inches respectfully; t) the end of the eighth platform on the east end of the north side scaffolds wan not guarded; u-w) one guard rail was placed at a 43.5-inch height on the ninth platform of the east end of the north side scaffolds instead of two guardrails at 21 and 42-inches respectfully; and x) where an employee was observed climbing onto and working from the inside cross braces of the second platform level of the scaffolds erected on the west end of the building.
