29 CFR 1910.134(g)(1)(i)(A):Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function: (a) Transit Mix 444 E. Costilla St, Colorado Springs, CO 80903: On or about December 15, 2021, and at times prior, employees were exposed to respiratory hazards such as respirable dust and silica dust. Employees in the batch plant wore an elastomeric half-face air purifying respirator with facial hair that came between the sealing surface of the facepiece. Abatement Note: Abatement certification is required for this item (see enclosed Certification of Corrective Action Worksheet).

29 CFR 1910.134(h)(2)(i):Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or were not packed or stored to prevent deformation of the facepiece and exhalation valve: (a) Transit Mix 444 E. Costilla St, Colorado Springs, CO 80903: On or about December 15, 2021, and at times prior, employees were exposed to respiratory hazards such as respirable dust and silica dust. A half-face elastomeric air-purifying respirator was found during inspection to be stored in a manner which did not protect it from contamination from respirable and crystalline silica dust.
