29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. On or about December 14, 2021, an employee was working on the ground below without head protection while other workers were conducting roofing operations, exposing the employee to the hazard of being struck-by falling objects.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. On or about December 14, 2021, five workers were securing shingles to the roof of a commercial building under construction up to approximately 30 feet high without fall protection, exposing the employees to a fall hazard.
