Employees engaged in welding activities worked at or near the outside edge of the structure and were not attached with a fall harness and lanyard or had any other means of fall protection. 29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. a. 730 Central Ave., Plainfield, NJ - School, north end The employer did not ensure ladders were extended at least 3 feet above the upper landing area and secured. Employees engaged in welding activities used a Werner, 24-foot extension ladder for access to the roof that was not extended 3 feet above the landing level and was not secured to ensure that ladder would not deflect when accessed. Violation observed on or about December 16, 2021
