29 CFR 1926.403(d)(1):Electric equipment was not firmly secured to the surface on which it was mounted: a) Lower Level - Employees were using (2) three way temporary outlets to supply power to extension cords and the outlets were laying on the floor, 120 volt, 15 amp service, on or about 12/16/2021. b) Lower Level - Employees were using a temporary service panel that was not secured and was suspended by the conductors, 120 volt, 100 amp service, on or about 12/16/2021. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Roof - Employees were removing sub roofing from a flat roof and not protected from a 22 foot and an 11 foot fall hazard, on or about 12/16/2021. b) Rear of Row House - Employees were walking across a lower level patio well on an 11 inch board and exposed to a 9 foot fall hazard, on or about 12/16/2021. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
