29 CFR 1926.601(b)(4):The employer had in use motor vehicle equipment having an obstructed view to the rear which did not have a reverse signal alarm audible above the surrounding noise level or an observer signaling it was safe to back up the vehicle. On or about December 8, 2021, a subcontractors employees were exposed to a struck by hazard while performing road and sewer line replacement on the 1500 block of Graham Road, Silver Lakes, Ohio 44224. A foreman for 1127 Construction Inc. was backing a Chevy 2013 3500 crew cab one ton pickup truck with a service body number UT 22 with an obstructed view to the rear and the employer did not ensure that he was using a back up alarm or ground spotter.
