The employer must ensure: (i)Each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following: (A) Covers; (B) Guardrail systems; (C) Travel restraint systems; or (D) Personal fall arrest systems. a) On or about November 30, 2021, employees, who were accessing sump pump pits such as but not limited to pit #4 in order to clean catch baskets and assist with removal of the covers and catch baskets during vacuuming, were not provided a fall protection system to prevent employees from being exposed to fall hazards of 4 feet 7 inches into 180 degrees Fahrenheit liquid such as but not limited to water, soybean oil, and soybean crude wastes. b) On or about December 1, 2021, an employee fell into the uncovered sump pump pit #4 in that the grates and catch baskets were removed exposing employees to fall hazards of 4 feet 7 inches into 180 degrees Fahrenheit liquid such as but not limited to water, soybean oil, and soybean crude wastes.
