29 CFR 1910.29(e)(1): The employer did not ensure that each cover for a hole in a walking-working surface is capable of supporting without failure, at least twice the maximum intended load that may be imposed on the cover at any one time. On or about December 9, 2021, the employer did not ensure that a skylight used to cover a hole in a roof was capable of supporting without failure at least twice the weight of any employee that might walk on it.
