29 CFR  1903.19(c)(1): Within 10 calendar days after the abatement date, the employer had not certified to OSHA (the Agency) that each cited violation has been abated, except as provided in paragraph (c)(2) of this section.   425 Worcester Road, Charlton, MA: As of  06/21/2022,  the employer had not certified to OSHA  that abatement was completed for the violations contained  in Citation 1,  item 1, Citation 1,  item 2, Citation 1,  item 3 and Citation 1,  item 4 that had  abatement dates of April 13, 2022, from Inspection number 1569216, issued on February 18, 2022 with a final order date of June 3, 2022.

Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.   a) 425 Worcester Road, Charlton, MA: On or about December 16, 2021: employee(s) working on a low slope roof engaged in roofing activities on a roof with unprotected sides and edges greater than 22 feet to the next lower level were not protected by either guard rails, safety net systems, personal fall arrest systems, or flagging and monitor systems exposing them to fall from height hazards.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  a) 425 Worcester Road, Charlton, MA: On or about December 16, 2021, employee(s) were exposed to fall hazards as part of their assigned duties and the employer had not trained them prior to exposure to these hazards in topics such as but not limited to recognition of fall hazards, dangers of fall hazards and procedures each employee must take to prevent or minimize risk of such exposures.
