29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d)(2): On or about December 15, 2021, and times prior thereto, at compression station, employer failed to conduct a adequate hazard assessment prior to performing general maintenance duties. As a result employee were exposed to inquires resulting in an amputation.
