In the electrical service room of building 3269 at University Place Apartments, Jacksonville, FL: On or about December 14, 2021, the employer exposed the qualified lead electrician and electrician helper to arc flash hazards, in that safety related work practices were not employed when the lead electrician changed out a 100 amp breaker within the energized 240VAC 600 Amp panel.

In the electrical service room of building 3269 at University Place Apartments, Jacksonville, FL: On or about December 14, 2021, the employer exposed the qualified lead electrician and electrician helper to electrical and arc-flash hazards, in that hazard risk category 1 electrical arc-flash protective equipment and arc-flash clothing hard hat, hearing protection, and rubber insulated gloves were not used when the lead electrician changed out a 100 amp breaker within the energized 240VAC 600 Amp panel.
