29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift. On or about December 17, 2021, at a construction site located on 910 Pine Market Ave in Montgomery, Texas , employees were exposed to fall hazards when accessing a steel structure to install sheet metal from a JLG aerial lift basket without fall protection system.

29 CFR 1926.760(a)(1):Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. On or about December 17, 2021, and times prior thereto, at a construction site located on 910 Pine Market Ave in Montgomery, Texas, employees were exposed to fall hazards exceeding 17 feet while welding on a steel structure without without a fall protection system.
