To abate this hazard, the employer must ensure that all exit route doors are able to be opened by employees from the inside at all times without keys, tools or special knowledge. 29 CFR 1910.22(a)(1): (1) All places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition. a) Storeroom, 918 W. Colonial Drive, Orlando, FL: On or about December 17, 2021, employees were exposed to slip, trip, and struck-by hazards while working in the main storage/receiving room. The storage room was obstructed in that there was no aisle or path for walking and employees were required to create an aisle as they went when retrieving merchandise and U-boats. Family Dollar was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1480377, citation number 1, item number 1, and was affirmed as a final order on 5/24/21, with respect to a workplace located at 181 Straight St, Paterson, NJ. Family Dollar was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1480368 , citation number 1, item number 1 and was affirmed as a final order on 5/17/21, with respect to a workplace located at 550 Getty Ave, Clifton, NJ. Abatement Certification and Documentation Required for this Violation.

On or about December 11,2021, employer did not report a fatality within the 8 hours allotted time. The employee succumb on December 11, 2021, employer knew on December 11, 2021, at approximately 9:00 PM, that employee was deceased and did not reported to OSHA until December 15, 20201, 8:18 PM.
