29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person prior to the start of work, and as needed throughout the shift, for evidence of situations that could result in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions. a.) Rest stop area 22, 1-75 S; On or about December 17, 2021, employer did not ensure the trench/excavation approximately 25 feet long, 4 feet wide, and 5 feet deep was inspected daily by a competent person to ensure the employees were protected from cave-in hazards.
