29 CFR 1910.213(d)(1):Circular handfed crosscut table saw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut. a) Warehouse, Shipping Department: On or about December 14, 2021, the employer exposed employees to amputation hazards, in that a "Grizzly Industrial, Inc." model G0771Z, 10 inch hybrid table saw was not guarded when used by employees in the fabrication of crates.

29 CFR 1910.305(g)(1)(iv)(A):Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure. a) Warehouse, Shipping Department: On or about December 20,2021, and at times prior, the employer exposed employees to electrical hazards, in that the employer did not ensure that a "Grizzly Industrial, Inc." model G0771Z, 10 inch hybrid table saw was directly connected to fixed wiring of the facility but instead supplied power to the saw via extension cords for employee use in the fabrication of crates.
