29 CFR 1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: At the jobsite located at 27161 David Hall Trail in Kirksville, Missouri an employee was exposed to an injury from a caught in hazard. The pipe threader operator was not trained on trained on the hazards associated with the equipment and proper methods of operation to avoid hazards. The employee was caught in the Rigid 300 pipe threader while operating the equipment.
