29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) DILKON MEDICAL CENTER AT 15 N 60 JUNCTION, DILKON AZ 86047: On or about December 1, 2021, the employer failed to report the in-patient hospitalization of an employee who fell from a portable ladder while performing cleaning operations. A report of the incident was not received by OSHA until December 20, 2021.
