29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees: (a) Iverson Chrysler Center, Inc. at 600 South Burr Street, Mitchell, SD 57301: On or about and at times prior to December 21, 2021, the employer did not ensure that the electrical connector on the main lift motor on Bay #13 was free of hazards in that the main electrical connector was damaged which exposed employees to the inner conductors. This condition exposed employees to electrical shock and fire hazards.

29 CFR 1910.334(a)(2)(ii):There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service: (a) Iverson Chrysler Center, Inc. at 600 South Burr Street, Mitchell, SD 57301: On or about and at times prior to December 21, 2021, the employer did not ensure that a retractable extension cord used within the area of Bay # 1 was removed from service due to the outer sheathing being damaged which exposed the inner conductors. This condition exposed employees to electrical shock and fire hazards.
