29 CFR 1910.119(i)(2)(ii):The pre-startup safety review did not confirm that prior to the introduction of highly hazardous chemicals to a process that the safety, operating, maintenance, and emergency procedures were in place and adequate: Employees engaged in ammonia refrigeration activities at the Lincoln Premium Poultry facility located at 1325 E Cloverly Rd, Fremont, Nebraska, were exposed to chemical burn and respiratory health hazards in that there was not a mechanical integrity program in place for process safety management covered equipment prior to plant start-up as required.
