The employer did not ensure that the object which directly injured the employee and the injury was accurately recorded in Column F of the 2021 OSHA 300 Log. Case 21-01 did not describe the object or substance that directly injured employee's lower back. b.) On and/or preceding July 19, 2021, a work-related injury occurred at the workplace. The employer did not ensure that the object which directly injured the employee and the injury was accurately recorded in Column F of the 2021 OSHA 300 Log. Case 21-02 did not describe the object or substance that directly injured employee's smashed thumb.
