29 CFR  1904.29(b)(1): You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year.  Sage Sustainable Electronics, at a jobsite located at 2801 Charter St., Columbus, Ohio 43228:  For the calendar year 2021 OSHA 300 log, employer had not included all information in column F as required for the following entries:  a.)   On and/or preceding February 8, 2021, a work-related injury occurred at the workplace. The employer did not ensure that the object which directly injured the employee and the injury was accurately recorded in Column F of the 2021 OSHA 300 Log. Case 21-01 did not describe the object or substance that directly injured employee's lower back.  b.) On and/or preceding July 19, 2021, a work-related injury occurred at the workplace. The employer did not ensure that the object which directly injured the employee and the injury was accurately recorded in Column F of the 2021 OSHA 300 Log. Case 21-02 did not describe the object or substance that directly injured employee's smashed thumb.
