On or about December 20, 2021, the employer did not provide adequate protective clothing or protective shields and barriers when employees came within 5 to 10 feet of the furnace to take temperature samples,  perform slagging operations, and collect samples, exposing employees to severe burn hazards from splashing molten iron.
