29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) On or about December 14, 2021,  and at times prior, employees operating the Komori L-268 printing press were not protected from rotating parts, point of operating and/or nip point hazards where the interlock function on the gates was not operational.  Exposure to this condition resulted in a press operator suffering hand injuries when the press rollers did not stop as the gates where opened.
