On or about December 27, 2021, during the operation of the MEDART #6 Two- Roll Straightener at the work location 2401 Gambrinus Avenue SW, Canton, OH 44706 (Plant A, Finishing Department). The employer did not ensure that the walking-working surface at the operator's station and around the machine was free from oil/coolant that had leaked on the floor in the working area exposing employees to slip and fall hazards.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are- barrier guards, two-hand tripping devices, electronic safety devices, etc. On or about December 27, 2021, during the operation of the MEDART #6 Two- Roll at the work location 2401 Gambrinus Avenue SW, Canton, Ohio 44706 (Plant A, finishing Department) the employer did not ensure operator(s) of the MEDART #6 Two-Roll Straightener were protected by one or more methods of machine guarding from the moving and rotating parts such as bar stock and rolls (point of operation): a. On or about December 27, 2021, the employer did not ensure operator(s) of the MEDART #6 Two-Roll Straightener were protected by one or more methods of machine guarding from the moving and rotating bar stock. The employer did not ensure safe feeding of bar stock into the unguarded point of operation and associated danger zone exposing employees to caught-on entanglement hazards likely to result in serious injury. b. On or about December 27, 2021, the employer did not ensure operator(s) of the MEDART #6 Two-Roll Straightener were protected by one or more methods of machine guarding from the rotating concave and straight rolls (point of operation). The employer did not ensure safe access while reaching into the machine to adjust the lubrication/coolant lever located directly above the point of operation, exposing employees' to caught-in hazard likely to result in serious injury.

29 CFR 1910.212(a)(3)(iii): Special hand tools for placing and removing material shall be such as to permit easy handling of material without the operator placing a hand in the danger zone. Such tools shall not be in lieu of other guarding required by this section, but can only be used to supplement protection provided. On or about December 27, 2021, during the operation of the MEDART #6 Two- Roll at the work location 2401 Gambrinus Avenue SW, Canton, Ohio 44706 (Plant A, finishing Department) the employer did not ensure hand tools, such as hand hooks, used to manually manipulate bar stock to ensure the bar stock landed on the guide plate of the MEDART #6 Two-Roll Straightener, prevented operators from placing their hands and arms in the unguarded danger zone of the machine's rotating parts, point of operation, and rotating bar stock.
