The employer did not ensure safe feeding of bar stock into the unguarded point of operation and associated danger zone exposing employees to caught-on entanglement hazards likely to result in serious injury.  b.	On or about December 27, 2021, the employer did not ensure operator(s) of the MEDART #6 Two-Roll Straightener were protected by one or more methods of machine guarding from the rotating concave and straight rolls (point of operation). The employer did not ensure safe access while reaching into the machine to adjust the lubrication/coolant lever located directly above the point of operation, exposing employees' to caught-in hazard likely to result in serious injury.
