29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about November 6, 2021, the employer failed to ensure that effective machine guarding was in place on the Barcelona Sash Punch named the Night Latch Limit Lock Punch. Barrier guarding was not in place to prevent employees from reaching into the orienting area where the hazard of moving machine parts exists.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On November 6, 2021, the employer did not report the the amputation of an employees right index finger to OSHA within 24 hours. The tip of the employees finger was amputated and OSHA was not notified until 12/22/21.
