29 CFR 1910.178(a)(5):The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: Location: site On or about 12/28/21, a Doosan fork lift model G25E-5 was equipped with six foot long fork extensions and used to lift pallets of material, and the employer did not ensure the forklift was marked to identify the fork extensions and show the approximate weight of he truck.
