29 CFR  1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  a. On or about December 28, 2021, and at times prior, in the Production Barn, the ingoing nip points and rotating parts associated with the lower conveyor belt and rollers of the first sticking conveyor of Production Line 3 were not guarded, thereby exposing employees to caught-in/caught-between hazards.  b. On or about December 28, 2021, and at times prior, in the Production Barn, the ingoing nip points and rotating parts associated with the lower conveyor belt and rollers of the second sticking conveyor of Production Line 3 were not guarded, thereby exposing employees to caught-in/caught-between hazards.

29 CFR  1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling.  a. On or about December 28, 2021, and at times prior, in the Production Barn on Line 3, the employer did not ensure that a relocatable power tap (power strip) was installed and used in accordance with its listing and labeling instructions. A relocatable power tap was permanently mounted and utilized with extension cords and another relocatable power tap plugged into it, thereby exposing employees to electrical shock, fire, and burn hazards.  b. On or about December 28, 2021, and at times prior, in the Production Barn on Line 3, the employer did not ensure that a relocatable power tap (power strip) was utilized in accordance with its listing and labeling instructions. A relocatable power tap was plugged into another relocatable power tap to power equipment, thereby exposing employees to electrical shock, fire, and burn hazards.

When normally enclosed live parts are exposed for inspection or servicing, the working space, if in a passageway or general open space, shall be suitably guarded.  a. 29 CFR  1910.303(g)(2)(i): Except as�elsewhere required�or permitted�by this�standard, live�parts of�electric equipment�operating at�50 volts�or more�shall be�guarded against�accidental contact by�use of�approved cabinets�or other�forms of�approved enclosures�or by�any of�the following�means:  a. On or about December 28, 2021, and at times prior, on Production Line 3 in the Production Barn, the employer did not ensure that live parts of electrical equipment operating at 208-230 volts were guarded against accidental contact in that an electrical box with live conductors had its door open. Employees were thereby exposed to struck-by electrical hazards.   b. On or about December 28, 2021, and at times prior, on Production Line 3 in the Production Barn, the employer did not ensure that live parts of electrical equipment operating at 120 volts were guarded against accidental contact in that electrical cords zip-tied along the bottom of the production line had exposed conductors. Employees were thereby exposed to struck-by electrical hazards.

On or about December 28, 2021, and at times prior, located in the Production Barn, the employer did not ensure that portable fire extinguishers were identified and readily accessible in that a fire extinguisher by the electrical panels behind Production Line 3 was blocked by storage of materials and equipment and was not identified, thereby exposing employees to burn hazards.
