29 CFR 1910.212(a)(3)(ii):The point of operation of machines whose operation exposed an employee to injury, was not guarded: a) The Andersons, Inc. on or about December 22, 2021, at the address of 415 Illinois Ave Maumee, OH: The employer did not ensure an employee operating the Wysong model D96 3 roll bending machine was protected from entering the point of operation. The employee operating the roll bender without the benefit of guarding was exposed to crushing hazards.
