On and about 12/28/2021, employees were exposed to slip hazards where there was water on workplace floors in the following areas:                                                                                    a.  Breezeway where water was on the floor across the aisle                                                                                   b.  Show Room Number 2 where there was water on workplace floor.

29 CFR  1910.176(a): Aisle(s) and passageway(s) were not kept clear and in good repair with no obstruction across or in aisles that could create a hazard:  6128 Bartmer Avenue, St. Louis: Employer is exposing employees to hazardous conditions where aisles are blocked.  The employees use mechanical material handling equipment. On and about 12/28/2021, employees were exposed to hazardous conditions where aisles were not kept clear.  Instances of the violations were:                                                                                a.  Warehouse where aisles were blocked by stored pallets of products where the employees used a forklift.                                                                                  b.  Show room 2 where a flexible cord was run across an aisle.

On and about 12/28/2021 an employee was exposed to struck-by hazards where the untrained employee was permitted to operate a forklift.

On and about 12/28/2022, employees were exposed to struck-by hazards where they operated a defective Yale forklift.  The forklift had damaged tires, damaged seat, and inoperative seatbelt.

On and about 12/28/2021, employees in Show Room 2 warehouse were exposed to electrical hazards where an energized power cord of refrigerator was connected to a black flexible cord.  The black power cord was used as substitute for fixed wiring.
