29 CFR 1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: On December 29, 2021 at the 11 Laurel street, east Hartford, CT job site: CSHO observed employees were removing roof shingles from the western section of the house (front of structure) with slope of approximately 6:12. There was a fall hazard to the ground below, about 11 (elven) feet from edge of the roof ridge to the ground below. Employees were not wearing a personal fall protection system (harness) or other means of fall protection.

29 CFR 1926.1053(b)(1):Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to the roof. On or about December 29, 2021 at the 11 Laurel Street, East Hartford, CT job site: The employer did not ensure that Little Giant ladder used to access the roof surface was extended at least three feet past the landing surface. This was a slipping and fall hazard for the employees who traveled the ladder up and down carrying roofing materials.
