29 CFR  1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  The employer is failing to protect employees from potential hazards associated with trenching operations; this was most recently documented on or about December 29th, 2021 in Gretna, Nebraska 68028.   The employer representative  located on-site during the inspection had not been properly trained in the identification of soil types,  prevention of unsafe conditions and the standards contained in the subpart.

29 CFR  1926.651(c)(2):A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  The employer is failing to protect employees from  cave-in hazards because there wasn't a ladder or other safe means of egress located in the trench- This was most recently documented on or about December 29th, 2021 at 180th and Cornhusker, Gretna, Nebraska 68028.  Multiple employees were observed crawling over the edge(s) of a trench.

The employer is failing to protect employees from slipping hazards.  This was most recently documented on or about December 29th, 2021, at 180th and Cornhusker, Gretna, Nebraska 68028.  An employee was observed standing in the inside of a vault in a unprotected trench that had approximately 2 -4 inches of water accumulation.
