29 CFR  1926.100(a):Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.  On or about December 29, 2021, at a commercial jobsite, employees worked without head protection on the ground below framing employees working from above. The employees was exposed to overhead struck-by hazards.

29 CFR  1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about December 29, 2021, and at times prior thereto, at a commercial job site, the employer did not ensure employees were provided with and used eye protection. Employees were exposed to eye injuries.

An employee was exposed to a fall hazard.

On or about December 29, 2021, and at times prior thereto, the employer did not ensure employees were provided a training program and formal training for each employee who used ladders .  Employees did not obtain the knowledge and skills to recognize and avoid ladder hazards and were exposed to fall hazards.
