29 CFR 1910.1200(h)(3)(i):The employee training did not include the methods and observations used to detect the presence or release of a hazardous chemical in the work area: (a) On or about February 10, 2022 and times prior, at or near 10747 Sand Creek Road and other well-sites located within the area, employees were not adequately trained to effectively interpret the reading of multi-gas meters and were exposed to atmospheric hazards such as but not limited to; elevated levels of carbon monoxide, oxygen deficiencies, elevated LEL, and/or presence of hydrogen sulfide.

29 CFR 1910.1200(h)(3)(iii): Employee training did not include the measures employees can take to protect themselves from chemical hazards, including specific procedures the employer had implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures and personal protective equipment to be used: (a) On or about February 10, 2022 and times prior, at or near 10747 Sand Creek Road and other well-sites located within the area, employees were not trained in the procedures for avoidance and/or evacuation when atmospheric hazards such as but not limited to; elevated levels of carbon monoxide, oxygen deficiencies, elevated LEL, and/or presence of hydrogen sulfide existed during work activities.
